Once the form has been filled in, you will be able to give it to your family, your G.P. and any Hospitals that you are already under so that they can be attached to your medical notes. We are trying to find a way to display that you have a "Will to Live." This would help to ensure that if disaster strikes while you are away from home, the people trying to help you would recognise that you have one and be able to act accordingly.
